Apprentice dispatch hits coder tool loop max iterations (60) and is killed with "lane-failed: coder tool loop exceeded max iterations (60)". Many tool calls had completed successfully beforehand — multiple a file path, a file path, a file path all landed, but partial. User has to triage what landed.
